  • Simple Life, LLC (“SimpleCare”)

  • Licensed healthcare providers and clinician groups who provide services through the SimpleCare technology platform and referral services

  • Business associates that support healthcare operations


SimpleCare operates as a technology platform and referral service that connects patients with independent licensed healthcare providers. Medical services are provided by those providers, who are responsible for clinical care and decision-making. The independent clinicians are not employees of SimpleCare.

We understand that your health information is personal and sensitive. We are committed to protecting your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable laws.
